Chapter 3: Exploration of Meanings

Once the patient is capable of maintaining working states in which intense, unpleasant emotions can be tolerated, the story of the trauma can be retold in a context of exploring personal meanings of the stressor events. Now intense feelings can be expressed in a titrated manner within the safety of the therapeutic alliance. Using a gradual reappraisal method, the previously overwhelming feelings are divided into subtopics. Feelings are transcribed from bodily sensations and images into words, communication, and negotiation.

The goals for the Exploration of Meanings phase of psychotherapy include:

• Desensitization using graduated, well-tolerated exposure to parts of the trauma story, while aiming to help the patient avoid reliving the traumatic events.

• Forming a new narrative structure focused on reaching new, more adaptive conclusions about the future of the self.
